Here is just one another free phproxy template by Proxy Stash. It is a high CTR template very attractive and sleek. The template is adsense ready, all you have to do is just upload and everything is done.
The zip folder contains a config.php which you have edit according to you requirements like Title, Meta keywords, Meta description, Adsense pub ID.

You can use the template for free but you must not edit/remove the links in the footer, by leaving the links in the footer it will help me in releasing more free templates.
